SYRACUSE, NY (WSYR-TV) — The Onondaga County Health Department has released its inspection reports for restaurants inspected during the week of March 26 through April 1.
Two food services failed their inspections:
- Tully’s Good Times, 520 Towne Drive, Fayetteville
- Oishi Fusion, 701 South Cruce Avenue, 2nd Floor, Syracuse

Tully’s Good Times in Fayetteville
Tully’s Good Times in Fayetteville had two violations, both serious.
Serious Violation: Food is not protected from contamination by other sources
The inspector found the sanitizing bucket stored next to the single service take-out container. This was corrected and the bucket was removed. The inspector also found two spray bottles of the chemical hanging on the rack next to and above the bag and box of soda. This was also modified and the spray bottles were removed.
Serious Violation: Improper cooling and refrigeration of potentially hazardous foods.
The inspector found four bowls of chicken wing dip, five bowls of crab dip, a large bowl of pico, a large bowl of diced tomatoes, and four bowls of chicken egg rolls all in the end-prep cooler at 50-60 degrees. was in Fahrenheit. The cook line employee stated that the temperature was at or below 45 degrees Fahrenheit at 10:30 a.m. and noted that the circuit was tripped at approximately 1:45 a.m. According to the inspector, the items had been refrigerated for an undetermined amount of time. This was corrected and the products were dropped.
Oishi Fusion
Oishi Fusion has 19 violations, 4 of which are serious.
Serious Violations: Food is not protected from contamination, temperature is not measured. (This violation was found twice)
The inspector found protein stored on top of raw fish and ready-to-eat vegetables (bok choy) in the walk-in refrigerator. This has been corrected and the operator has re-arranged the walk-in cooler, providing education on where it goes. At the sushi station, raw fish products were also stored on top of avocados. This has been corrected and all prepared foods have been removed from the cooler.
Serious Violation: Improper cooling and refrigeration of potentially hazardous foods.
The inspector found that several foods were improperly stored in the cook line flip-top cooler so the temperature was not maintained below 45 degrees Fahrenheit for an undetermined amount of time. The food was wrapped in paper towels and stored in heavy plastic containers inside the metal pan of the console. This was corrected and approximately 0.5 pounds of cooked pork and 4 ounces of sarimi steak were voluntarily discarded. Education is provided and food safety literature is provided in the operator’s primary language.
Serious Violation: Improper cooling and refrigeration of potentially hazardous foods.
Inspector finds approximately 3 pounds of chicken breast cooked to 79-85 degrees F in fry basket and ready for plating. Final cooking time could not be determined. This was corrected and 3 pounds of cooked chicken breasts were voluntarily discarded. Education is imparted
Food is generally not protected: The inspector found the storage space behind, dry goods are stored uncovered in bulk storage. They also found that large amounts of chicken were stored in non-food grade plastic inside the walk-in refrigeration and that grocery bags were used to store food in the refrigeration unit, not food-grade plastic bags. In the rear preparation area and rear dry storage, fly strips were also present and could contaminate nearby equipment and packaging.

Poor hygiene and activities of food workers: The inspector found in the rear prep space, an employee spinning shrimp while their hair was open and an employee eating next to the cook line.
Poor sanitary design, construction, equipment and vessel installation: The inspector found cut boards in poor repair on the cook line with deep spots and stains.
Poor sanitary design, construction, equipment and vessel installation: Inspector found multiple wire shelves in refrigeration and dry storage with corrosion. The glass door refrigerator also had gasket material and the chest freezer was in poor repair, damaged and torn.
Improper cleaning, washing and sanitizing of equipment and utensils: The inspector found that there were no sanitation buckets near food preparation surfaces during the inspection.

The flip-top cooler surface also had greasy residue and food debris. There was also residue and debris on the shelves of the walk-in refrigeration.
Inadequate pest and rodent control: Inspector found black flag flying insect spray in chemical storage area.
Improper construction and maintenance of physical facilities: The inspector noted that there was debris and residue on the floor throughout the facility.

Many places passed their inspection between March 26 and April 1.
